The case stemmed from a series of missed opportunities to diagnose lung cancer in August 2016 and December 2017. The radiologist incorrectly interpreted chest x-rays showing right upper and right lower lobe opacities, dismissing them as mere pneumonia or hemorrhage rather than conducting further investigation into potential malignancy. A subsequent CT scan in December 2017 revealed a pulmonary nodule, but again, appropriate follow-up imaging was not recommended despite clear warning signs that warranted immediate attention.
